Overview

NaphCare is hiring a Discharge Planner just like you to join our team at the Pima City Jail located in Tucson, AZ. Pay rates vary based on shift, site needs, and experience. Shifts and hours vary based on coverage needs and can be discussed upon interview. PRN shifts and hours vary based on coverage needs for FT shifts and will be determined by site leadership and prn employee availability.

Responsibilities

Responsibilities for the Discharge Planner:

  • Conduct rounds with the physicians and mental health staff to meet with patients to discuss placement options, organize records for transition, and provide advice regarding community assistance organization, transportation options, etc
  • Involve patient in discharge planning
  • Assist patients with the identification of a support network, instructions for future care and follow-up
  • Assist patients with completion of all clinical and financial documentation needed for social service organizations
  • Coordinate with healthcare and security staff regarding patients' release dates
  • Liaison for all referrals for involuntary examinations or crisis stabilization
  • Liaison for all inpatient care management


Qualifications

Qualifications for the Discharge Planner:

  • Must hold and maintain a Bachelor's Degree in a human service field
  • A valid CPR card
  • Must have knowledge and experience in the development of mental health discharge plans and coordination with community service providers.
