Description Under the direction of the Sepsis Nursing Supervisor at Providence Mission Hospital, the Sepsis Nurse Navigator works with the care team to implement program goals and objectives by providing culturally competent and linguistically appropriate care. The Sepsis Nurse Navigator faciliates the care of patients with Limited English Proficiency (LEP) with the diagnosis of sepsis across the heathcare continuum. He/she provides support to LEP patients and their families with a focus on providing education, enhancing the communication with the care team, and facilitating discharge planning and continued follow up and support as needed through 30 days post-discharge. The Sepsis Nurse Navigator collaborates with community health/nurse navigators at the Family Resource Centers (FRCs), community health clinics, community organizations and groups to support and promote the health and wellness of the whole person body, mind and spirit. He/She provides health education and health related services including one on one teaching, classes, health advocacy, health access support groups, case management and outreach to identified underserved populations. The Sepsis Nurse Navigator will tailor services to the unique health and wellness needs of the individual/family served. Being sensitive to cultural and belief system differences, utilizing good communication skills, the Sepsis Nurse Navigator initiates and cultivates positive working relationships with hospital staff and individuals. She/he is self-directed and able to work independently in meeting program goals and needs of communities.
