St. Ann's Community is currently looking for compassionate and energetic CNA's! St. Ann's Community has built a reputation for providing Rochester's most exceptional care for our Elders. We offer a wide range of positions at our Irondequoit and Webster campuses.

St. Ann's Community Offers:

* Full time Day & Evening Shifts
- Starting rate - $18.50/hr.
* $4/hr. Evening/Night Differential
* $6/hr. All Weekend Shifts Differential

Job Description:

* Provide high quality, individualizes patient care in collaboration with our nursing team

* Take initiative while assisting and maintaining patient care

* Assist the resident in activities necessary as per individualized care planning

* Transport Elders to and from activities

* Perform evaluation and reporting

* Provide resident and family with emotional support individualized care planning

* Receive/give reports to team members/team leaders to appropriate professional staff

Candidate Requirements:

* Valid NYS Certified Nursing Assistant License (CNA) - Required

* Minimum of 1 year experience working with Elders - Preferred

* Ability to work in a team oriented work group

* Proficient in various computer programs and Medical Records Documentation system
