About the Role

HouseCalls brings preventive care directly to health plan members through annual in-home visits. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you'll perform comprehensive health assessments that help identify unmet needs, support ongoing care management, and improve overall health outcomes.

This is a non-prescribing, field-based position focused on building meaningful patient connections, providing health education, and partnering with primary care providers to ensure members receive the care and support they need.

Primary Responsibilities
  • Complete comprehensive in-home health assessments, including:
    • Medical history review
    • Medication reconciliation
    • Vital signs and physical examinations
    • Evidence-based screenings and point-of-care testing, when appropriate
  • Identify and document clinical conditions to support care planning
  • Share assessment findings with primary care providers to help close gaps in care
  • Recognize urgent or emergent situations and take appropriate action
  • Educate patients on disease management, medications, and treatment adherence
  • Assess social determinants of health and connect members with community resources as needed
