| Position Highlights |
The Health Mentor will provide recovery-oriented peer support and skills training services, teaching classes and providing mentoring sessions to assist community members with serious mental illnesses, substance use disorders, and/or general mental health conditions to increase their knowledge, skills and motivation to improve their health, reach their recovery goals, live independently, and integrate in their communities. The Health Mentor will also participate in community outreach recruitment events and activities.
- This position requires current certification as a Peer Recovery Support Specialist, eligible to provide Peer Support Services in the state of Arizona.

| Duties & Responsibilities |
- Provide person-centered health mentoring, skills training, peer support, and recovery-oriented assessment, motivational interviewing, and goal setting services, and work within protocols to maintain a clean, safe, welcoming recovery-oriented environment. Teach a variety of classes in accordance with the HWC curriculum, including but not limited to: Nutrition, Cooking Skills, Physical Activity, Stress Management, Mindfulness, Tobacco Cessation, etc.; and provide 1:1 peer support services.
- Complete required documentation in compliance with CSA certification, RBHA and AHCCCS requirements, such as progress notes, monthly summaries, assessments, incident reports, etc., maintaining CSA certification requirements, including required trainings, TB clearance, etc.
- Collaborate with team members, students, alumni, and members of adult recovery teams for coordination of care, delivery of services and quality improvement activities. Includes attending in person and online meetings, providing transportation in a 15-passenger van for community-based services for community-based activities, and providing first aid and coordination of care related to medical emergencies.
- Participate in outreach recruitment presentations and activities, including Arizona Palooza and other community events.

| Minimum Qualifications |
- Bachelor's degree or equivalent advanced learning attained through experience required.
- One year of relevant work experience.
- This position requires current certification as a Peer Recovery Support Specialist, eligible to provide Peer Support Services in the state of Arizona.
