Luther Manor is seeking a Wound Treatment Nurse. The Wound Treatment Nurse is responsible for assessing, treating, monitoring, and preventing wounds and skin integrity issues across the resident population. This role ensures adherence to evidence-based clinical practices, regulatory requirements, and facility protocols to promote wound healing, prevent avoidable pressure injuries, and improve resident outcomes. The Wound Treatment Nurse collaborates with the interdisciplinary team to develop individualized treatment plans, ensure accurate and timely documentation, and provide education and clinical support to nursing staff. Essential Job Functions: * Perform comprehensive skin and wound assessments on admission, weekly, and with any change in condition * Evaluate and manage acute, chronic, and complex wounds, including pressure injuries, surgical wounds, and skin tears * Develop and implement individualized wound treatment plans in collaboration with providers * Perform and/or oversee wound treatments in accordance with physician orders and facility protocol * Monitor wound progression and identify early signs of deterioration or infection * Notify providers promptly of changes in wound condition or lack of healing progress * Ensure all wounds and risk factors are accurately reflected in the resident care plan * Participate in interdisciplinary team (IDT) meetings, wound rounds, and care conferences * Maintain accurate, timely, and survey-ready documentation in the electronic medical record (EMR) * Ensure appropriate staging, measurements, and classification of wounds, including present on admission (POA) identification * Identify residents at risk for skin breakdown and implement prevention interventions (e.g., repositioning, offloading, moisture management) * Ensure appropriate support surfaces and equipment (mattresses, cushions, heel protectors) are in place and functioning * Provide education to nursing staff and CNAs on wound care practices, prevention strategies, and documentation expectations * Serve as a clinical resource to staff regarding wound care protocols and product use * Maintain appropriate inventory and utilization of wound care supplies * Participate in QAPI initiatives, including tracking and trending wound outcomes and contributing to root cause analysis for facility-acquired pressure injuries * Support onboarding and competency validation for wound care practices Knowledge, Skills and Abilities: * Strong clinical knowledge in wound assessment, staging, and treatment * Knowledge of pressure injury prevention strategies and support surface use * Ability to interpret clinical changes and respond appropriately * Strong documentation and attention to detail * Effective communication and collaboration with interdisciplinary teams * Ability to educate and support staff in clinical best practices * Familiarity with regulatory requirements (CMS, state guidelines) related to skin integrity and wound care * Proficiency in electronic medical record systems
