General Summary: Develops and coordinates educational programs for assigned staff, and provides direct care for patients in their place of residence. Acts as a resource person to professional staff members on case load management and the availability of community resources. Monitors the quality of services provided to patients and performs related duties as required. EDUCATION & EXPERIENCE: Requires graduation from an accredited school of nursing. BSN preferred. Requires current licensure in the State of Michigan as a Registered Nurse. Requires a minimum of one (1) year of Home Health Care or Hospice clinical experience. Requires reliable transportation, a valid driver's license, and an acceptable driving record. Requires the ability to lift, carry, transport or move patients as needed.
