Position Overview
The Access Navigator for Community Integration & Recovery plays an essential role in supporting the administrative and coordination needs of recovery-based, transition-focused, and community integration services. This position ensures efficient documentation, supports communication across multidisciplinary teams, and assists in coordinating care to help individuals transition to the appropriate level of community living and behavioral health supports. This is a highly organized, detail-oriented role that directly contributes to smoother workflows, better client outcomes, and enhanced service coordination.
Core Responsibilities
- Scan, upload, and organize client documents into the electronic health record
- Complete declination letters, discharges, and required tracking
- Obtain medical records and collect missing IATP signatures
- Assist with completing consents, financials, ROIs, and obtaining client signatures
- Collect and track data for grants, compliance, billing, and quality improvement
- Run and manage program-specific reports to support operations and performance goals
- Schedule staffings, follow-up meetings, and support team calendar coordination
- Attend team meetings, take minutes, and distribute notes to attendees
- Support clinical operations through administrative coordination
- Maintain accurate documentation and electronic workflows
- Perform other duties as assigned or negotiated with supervisor
Knowledge, Skills & Abilities
Knowledge
- Understanding of document management, clerical workflows, and EHR systems
- Knowledge of supportive housing, recovery, or behavioral health services (preferred)
- Familiarity with case management processes and tracking requirements
Skills
- Strong organizational and time-management abilities
- Skilled in electronic document management, data entry, and scheduling
- Proficiency with EHR systems, digital signature tools (e.g., DocuSign), Microsoft Office, and Google Workspace
- Strong written communication skills, including documentation and note-taking
- Effective verbal communication with staff, clients, and community partners
- Strong problem-solving skills and follow-through
Abilities
- Ability to maintain accuracy and attention to detail
- Capacity to work collaboratively across departments and with external partners
- Ability to adapt to changing priorities and workflow processes
- Maintain compliance with privacy standards and handle sensitive information
- Provide professional, empathetic support to individuals with behavioral health conditions
- Organize and track data for grants, billing, compliance, and reporting
- Represent the program and agency professionally
- Exercise sound judgment and escalate concerns appropriately
Qualifications
- High school diploma or equivalent
- Minimum 21 years of age
- Valid driver's license, reliable transportation, and proof of auto insurance
