Overview
In collaboration with multidisciplinary diabetes management team provides diabetes/nutritional self-management education (DSMES) to patients/families living with diabetes.
Responsibilities Essential Duties and Responsibilities
Education and Experience Bachelor's degree from a four-year college/university in Dietetics/Food and Nutrition. Certification/Licensure State certification/licensure as a Registered/Licensed Dietician in WI and IL. CDCES certification required. If not CDCES certification eligible or not CDCES certified at hire, 15 CME credit per year required. CDCES certification required within 2-years of becoming eligible or being hired.
