SIGN-ON BONUS OFFERED!!! Preceptor Home Health & Hospice is growing! We are a home health, hospice, and palliative care agency providing services in patient homes in 15 counties throughout Wisconsin. Patient homes include private residences, assisted living facilities, memory care units, and any other place they may call home. Preceptor strives to provide high quality care through our multifaceted programs. We have branch offices located in Germantown, Fond du Lac, Waukesha, Sheboygan, and Appleton. We are currently in need of a Full-Time Home Health Certified Occupational Therapy Assistant for our Milwaukee and Waukesha County Territories. SIGN-ON BONUS UP TO $2,000, PAID IN INSTALLMENTS! Full-Time, 32-40 hours per week Job Requirements: Carry a current COTA licensure in the State of Wisconsin Excellent communication skills with patients, families, facility staff, doctors, and teammates Ability to work independently Good organization and effective time management skills Willingness to travel to meet patient needs Compliance with documentation standards and regulations Prior home health experience preferred, but does not affect eligibility to apply Additional Information: Health, dental, vision, and life insurance offered Flexibility in scheduling to aide in work life balance Paid mileage Accrued PTO and sick time 6 paid holidays/year 401K with employer contribution We would love the opportunity to add you to our team!
