Summary:

The social work practice that occurs in hospitals and other health care settings to facilitate health, prevent illness and aid physically ill patients and their families to resolve the social and psychological problems related to the illness. Social work also sensitizes other health care providers about the social psychological aspects of illness. The Social Worker provides clinical coordination and therapeutic interventions through assessment, coordination, referral and Interdisciplinary planning for high risk population. The Social Worker also links people to systems that provide them with resources, services and opportunities to achieve and maintain maximum functioning. Acts as a Social Worker Field Instructor for BSW/MSW students. Provides counseling and support to patients and families regarding adjustments to disease process and illnesses.
