Summary

Under the direction of the Director of Nursing, in cooperation and collaboration with the interdisciplinary team, the Nurse Manager is to supervise the day-to-day nursing activities of the Sub-Acute Rehab unit. Such supervision must be in accordance with current federal, state and local standards, guidelines and regulations our established policies and procedures, and as may be directed by the Director of Nursing and/or Administrator to ensure that the highest degree of quality care is maintained at all times.

Essential Job Responsibilities

  • Staff will provide patient care, treatment, and services within the scope of their license, certification or registration and as required by law and regulation.
  • Supervise all nursing care in the facility or the units to which assigned by demonstrating the knowledge and clinical expertise required
    • Review and implement all nursing procedures and systems relative to the unit(s)
    • Assess and evaluate the systems which facilitate the delivery of quality, evidence-based patient care
  • Responsible for direct supervision of sub-acute rehabilitation nursing department
    • Supervise activities of all certified nursing assistants
    • Provide direction and oversight to staff nurses
    • Assess and evaluate the performance and competency of staff
    • Provide one to one education to nursing staff as needed to improve level of care, documentation, and adherence to state and federal regulations and Mary Free Bed policies and Standards of Behavior.
    • Lead and participate in staff meetings
    • Evaluate, develop, mentor, coach, counsel, and discipline department staff in accordance with Mary Free Bed policies.
  • Monitor documentation daily, audits medication records and rooms, treatment records, and nurses notes to assure treatments are rendered accurately and documentation reflects current resident needs and status; ensure follow-up on documentation as needed
  • Conduct rounds several times per shift to evaluate the quality of care provided, identify changes in condition, and assist patients and staff as needed to address care or service needs
  • Participate in patient care conferences with interdisciplinary team; lead nursing elements of patient discharge planning, patient and family education, and post-discharge care planning
  • Responsible for ensuring development and implementation of individualized care plans for all patients.
    • Evaluate the patients' responses to nursing interventions
    • Select and institute appropriate nursing intervention, which might be required to stabilize a patient's condition and/or prevent complications
  • Assure regulatory compliance is met in all aspects of nursing care
  • Assists Director of Nursing and participates in the Quality Assurance and Process Improvement in relation to incident reporting to assure complete documentation of occurrences. Analyzes data and develops appropriate action plans, identifies trends, patterns and outcomes. Provides these plans and trends to various committees attended.
  • Fulfill responsibilities in Nursing Leadership on-call rotation as assigned.
  • Under the direction of the Director of Nursing/Administrator, assist with and/or independently complete other duties as assigned.
  • May be required to work beyond normal working hours, on weekends and other positions temporarily when necessary and may be subject to call back during emergency conditions or State/CMS survey activity. May be required to work shifts other than those for which normally assigned.

Customer Service Responsibilities

Demonstrate excellent customer service and standards of behaviors as well as encourages, coaches, and monitors the same in team members. This individual should consistently promote teamwork and direct communication with co-workers and deal discretely and sensitively with confidential information.

Essential Job Qualifications

  • Must have a current license to practice as a nurse in Michigan. Registered Nurse (RN) preferred; Licensed Practical Nurse (LPN) also accepted. Current CPR certification is mandatory.
  • A minimum of two years' experience in long term care setting and prior management experience preferred.
  • Must have excellent assessment, planning, organization, communication and interpersonal skills with an understanding of person centered care.
  • Ability to work flexible shifts including periodic weekends, holidays and off shifts.
  • Excellent verbal and written communication and possess the ability to express self clearly and effectively in an interdisciplinary care setting with residents, families, and staff members.
  • Ability to effectively present information and respond to questions from patients, staff, families, vendors, and managers.
  • Ability to define and solve problems, collect data, establish facts and draw valid conclusions and produce action plans.
  • Ability to interpret an extensive variety of technical journals, government requirements and regulations.
