Job Summary

The Medical Social Worker plays a vital role in enhancing patient well-being by addressing the social, emotional, and practical needs of patients and their families. This role emphasizes empowering patients who are vulnerable, oppressed, or living in poverty. Medical Social Workers collaborate closely with physicians, nurses, utilization management, discharge planners, Clinical Case Management, and other allied health professionals to support comprehensive patient care.

Responsibilities:

  • Conduct psychosocial assessments to identify patient and family needs, including financial, emotional, and social support.
  • Develop and implement individualized care plans to address identified needs and enhance patient outcomes.
  • Provide counseling, crisis intervention, and supportive services to patients and families.
  • Coordinate resources and referrals for community services, government assistance programs, and internal hospital resources.
  • Collaborate with multidisciplinary teams to facilitate smooth patient care, hospital discharge, and transitions to other care settings.
  • Advocate for patients' rights and needs within the healthcare system and the community.
  • Assist with advance care planning, end-of-life decision-making, and other sensitive patient discussions as appropriate.
  • Maintain accurate and timely documentation in accordance with hospital policies and regulatory requirements.
  • Participate in quality improvement initiatives and interdisciplinary team meetings to enhance patient care and departmental processes.

Minimum Qualifications - For full consideration, applicants are encouraged to upload a license and/or certification if required for the position

  • Master's degree in Social Work from a CSWE-accredited institution
  • A minimum of two years of postgraduate experience in the field
  • Experience or knowledge of healthcare or health and human services, including child welfare
  • Psychosocial Assessment and care coordination skills

Preferred Qualifications

  • Applicant must have an ACSW number or be eligible to receive one
  • Experience working in an acute care setting

Department Overview

The Clinical Social Services and Case Management Departments at UC Davis Health have the combined responsibilities of twenty-four-hour hospital-wide Utilization Review, Discharge Planning, TAR, Medicare patient notification letters, initial and concurrent authorizations, and hospital-wide Health Care Management. The Clinical Social Services Department provides comprehensive social services to patients and their family members, who possess a variety of biopsychosocial needs, during their hospitalization.
