Position Title: Care Manager

Work Location: Pinellas and Pasco County, FL

Assignment Duration: 3 months (Possibility of extension and conversion)

Work Schedule: Monday through Friday 8am-5pm

Work Arrangement: Remote (Member - facing)


Position Summary:

Manage a weighted caseload of 200 members, primarily BH cases, including siblings and some medical needs.

Background & Context:

* The Region 5 Team is a team comprised of RNs, LPNs, LCSWs and LMHCs that manage a weighted caseload of 200 members. A majority of the caseload will be BH cases that may include the sibling and some medical needs.

* CMS is seeing an increase in members with complex BH needs and thus the need for hiring more BH health related staff to provide support to our members and families.

Key Responsibilities:

* Day to day operations as a BH Case Manager; same expectations as the FTE position

* Reports directly to Supervisor and Manager, attends regional team meetings on a weekly and/or monthly basis.

* The BH CM will manage the needs of their assigned caseload, attend any staffings, assist in coordination of care, work collaboratively with any providers/stakeholders involved with the member and their family and be able to document succinctly in our case management platform all interactions and correspondence.

Qualification & Experience:

* Required: Social Work, Psychology, Mental Health, Counselor Education

* Required: LCSW or LMHC

* Must haves: Excellent communication skills, organization skills, and critical thinking skills.

* Nice to haves: Bilingual, previous work with DOH, CMS, MMA, DCF, or CBC

* Disqualifiers: < 2 years pediatrics

* Performance indicators: able to multitask, manage high needs members, manage children and family members with mental health illness; provide excellent customer service; partner and work collaboratively with all providers/stakeholders. Manage challenging cases with multiple barriers.

* Top 3 must-have hard skills:



  1. Communication, organization, and skilled with technology
  2. 2 years or greater of pediatric experience
  3. Nice to haves would put the candidate at the top of the list.
