Salary: $72,000 to $77,000 per year DOE

This is a fully Work From Home opportunity

SHIFT: Multiple Shifts available weekday and weekend all Full Time

Previous Home Health or Hospice experience preferred

HomeCare Home Base experiece preferred


What You Need to Know

Job Duties:

  • Responsible for the quality of home health patient Care Plans in collaboration with assessing clinician and coding coordinator.
  • Utilize referral documents, orders, comprehensive assessments, and OASIS documentation to edit/build a detailed and individualized POC that demonstrates home health eligibility and skilled need.
  • Maintain up to date status information on any assigned cases that are unable to be completed due to outstanding information.
  • Notify leadership of problematic trends identified during case reviews.
  • Maintain professional and technical knowledge by attending educational workshops and reviewing professional publications.
  • Assist with other chart audits/tasks as assigned.

Qualifications

Be the Best RN Plan of Care Coordinator Manager You Can Be.

If you meet these qualifications, we want to meet you!

  • An associate degree in nursing with current unencumbered licensure to practice nursing in the state of residence required. Bachelor's degree preferred.
  • Minimum 1 year experience in the health care required. Home health experience preferred.
  • 1 year HCHB experience strongly preferred
  • Oasis Certification will be requried for this role.
