Department:REP | CS South Central Pine BluffDepartment's Website: Summary of Job Duties:We are seeking a Licensed Practical Nurse II (LPN II) Care Coordinator to join our team at the UAMS Family Medical Center in Pine Bluff, AR. This position plays a vital role in ensuring seamless patient care by supporting clinical staff, managing patient communications, and assisting with care coordination activities. Under supervision, the LPN II Care Coordinator will perform previsit planning for Transitions of Care visits, ensuring readiness and supporting care gap closure. The coordinator will provide essential information during huddles to clinical staff and physicians, return patient phone calls, conduct phone triage, refill medications as directed, assist with referrals, and coordinate the acquisition of durable medical equipment (DME). The position also involves managing correspondence received via fax, email, postal service, or patient delivery and filling in for absent nurses or other clinic personnel as needed.The LPN II Care Coordinator must be able to perform all duties within the scope of a Licensed Practical Nurse and support patient care activities as required. This includes demonstrating strong organizational and multitasking skills, effective communication abilities, and attention to detail. The ideal candidate will have problem-solving capabilities, the ability to work under supervision, and proficiency in patient care coordination and administrative tasks.
