Home Care Coordinator Assistant Location: Johns Hopkins Bayview Medical Center and Johns Hopkins Hospital Hours: 8:30a-5p Pay rate: $24 per hour Requirements The Home Care Coordinator Assistant will assist in all functions related to the JHHCG's Home Care Coordinator and posses an understanding of home care products and services and the services and resources available in the community. Verify insurance and provide needed information to agency and other patient information. Review referral packet to ensure all required information is present. Effectively communicate with agency (verbally and electronically) to ensure all information is provided to safely initiate care (referral, attachments, medication list, physician orders, and discharge instructions) and confirm receipt of referral with receiving agency. Ensurepatient discharge occurred and if not, follows up with agency for delays or changes. Provides agency any necessary follow-up to ensure patient care is provided as anticipated. Notifies HCC & team once arrangement confirmed. Maintain positive relationships with unit staff and outside agencies. Input HCC information into HCC flow sheet accurately and timely. File log sheets and appropriate clinical documentation so that information is retrievable and well-organized.Completes time studies completely and timely. Maintain updated master resource manual for office including accurate agency lists and ensures HCC has inventory. Qualifications High School Diploma or GED. One to two years data entry and customer service experience in a health care setting to include payor verification and authorization procedures. Excellent interpersonal and organizational skills, with the ability to multi-task, including written and verbal communication skills. Knowledge of medical terminology Ability to travel between multiple inpatient/outpatient hospital settings.
