Care Management Extender

Scope/Position Summary
Care Management is a team process that includes assessment, planning, facilitation and evaluation of the services required to meet the patient's needs through the healthcare system. The care Management Extender will provide programmatic support services for Care Management areas covering UH, Ross, Brain and Spine, and Dodd Hall. This position will be responsible for coordinating CM/SW program support inclusive of throughput/capacity management and discharge planning.

Minimum Qualifications
Bachelors degree or an equivalent combination of education and experience required; Experience in Acute Care Hospital Setting, experience in assisting with patient care operations preferred Proficient in Microsoft Office (Word, Excel, Power Point, Outlook) Experience with EPIC preferred, Customer service skills

This position may include working 1 weekend day as a part of the standard schedule.
