Provides leadership, direction and administration of clinical operations for the CareCoach Connect operations to ensure accomplishment of program and organizational objectives. Directs, supervises and coordinates the overall operation of the program and staff. Sets priorities for the team to forecast and plan activities to ensure appropriate care is provided to the patients. Identifies and resolves operational problems using defined processes, expertise, and judgment. Provides strategic oversight to program operations in collaboration with the Associate Medical Director. Develops and executes collaborative strategies for program growth within the market. Primary Responsibilities:
- Partner with the Associate Medical Director as your dyad partner and take accountability for all of the operational tasks
- Make sure that our hospitalized patients are getting their transition visit within a timely manner
- Lead and support operationally the Interdisciplinary Team and Market Ground Ops Meetings
- Resource manager for everyone in your Pod to include but not limited to: onboarding, training, eCW, schedules, etc.
- Accountable for the pod performance that includes productivity, full schedules, and quality metrics
- Attending weekly Pod IDT and daily Pod meetings, for assigned Pods
- Directs, supervises, coordinates specific functions and activities including systems, policies and procedures, human resources, support services, etc. required
- Assists in the development and marketing of new services and referral sources
- Ensures that trainings and certifications are up to date and complete for employees
- Facilitates the employment, development and management of employees
- Promotes the provision of cost-effective, high-quality health care services to patients through the monitoring of clinical performance
- Cooperates with medical and administrative staff to ensure compliance with standards and regulations
- Ensures supplies are appropriately stocked for employees at the corporate location and the individual field staff
- Ensures region is well promoted and properly represented through marketing and community relations
- Provides continual process improvement including all clinic processes and/or services
- Communicates with Director/VP to discuss barriers, accomplishments, and issues encountered
- Participates in clinic communication and public relations programs
- Maintains professional affiliations and enhances professional development to keep pace with the trends in health care administration
- Responsible for maintaining weekly checks with their direct staff and monthly formal one on ones
- Performs other related duties which may be inclusive, but not listed in position job description
- Completing equipment inspections to ensure maintenance is complete and all equipment are in working order

- 2+ years of healthcare administration/operations experience including one year of supervisory experience
- Proficiency in Microsoft Word, Excel, Outlook, Teams, PowerPoint applications
- Knowledge of health care administration, clinic philosophy and policies and operating procedures
- Knowledge of human resource management practices including supervision and staff development
- Knowledge of governmental regulations and reimbursement standards
- Skill in exercising initiative, judgment, problem solving, decision making
- Skill in developing and maintaining effective relationship with staff, Senior Management, patients, vendors, and payers, public
- Skill in organizing work to achieve clinic goals and objectives
- Ability to analyze and interpret complex data
- Ability to research and prepare comprehensive reports
- Ability to monitor compliance standards
Preferred Qualifications:
- Current unrestricted Vocational Nurse license for applicable state of employment
- Professional healthcare license or certification
- Supervisory experience within a clinic setting
Physical & Mental Requirements:
- Ability to lift up to 50 pounds
- Ability to push or pull heavy objects using up to 100 pounds of force
- Ability to stand for extended periods of time
- Ability to use fine motor skills to operate equipment and/or machinery
- Ability to properly drive and operate a company vehicle

- Occasionally requires exposure to communicable diseases or bodily fluids
The salary range for this role is $89,800 to $176,700 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
