GENERAL SUMMARY:

At the direction of the Registered Nurse (RN) and/or Licensed Practitioners, the MMC LPN Staff contributes to meeting patient's priority needs by assessing and participating in interdisciplinary care planning, providing delegated care and timely communication of the patient's response. The MMC LPN Staff contributes to an efficient and effective work environment through activities including professional communication and documentation.

PRINCIPAL JOB FUNCTIONS:

  • Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
  • Performs service excellence must-haves to achieve an excellent patient/customer experience.
  • Uses quality improvement processes, programs or outcome to help improve department operations.
  • Works collaboratively with the RN and provider to provide appropriate nursing care as dictated by licensure to assigned patients.
  • Assists in the collection of comprehensive data including physical, functional, psychosocial, emotional, cognitive and cultural assessment in a systematic and ongoing process while honoring the uniqueness of the person.
  • Assists in the identification of appropriate nursing diagnosis.
  • Executes therapeutic and/or supportive interventions under the direction of the RN.
  • Participates in the development, implementation and evaluation of the plan of care.
  • Assists with the patient's transition to next level of care.
  • Utilizes appropriate resources to plan and provide nursing services that are safe, effective and financially responsible.
  • Collaborates with the patient, family, providers and others in the provision of nursing care.
  • Assists in providing education for the patient and family according to their needs, abilities, preferences and readiness to learn.
  • Provides care in conjunction with other members of the interdisciplinary team.
  • Attends monthly staff meetings as well as completes all mandatory training.
  • Performs other related projects and duties as assigned.



EDUCATION AND EXPERIENCE:

Graduation from a School of Practical Nursing with current licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act. Must provide evidence of certification and/or instruction for practice area as required by State or Federal standards/regulations.

OTHER CREDENTIALS / CERTIFICATIONS:

Current Basic Life Support (BLS) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
