Title: Care Manager

Duration: 03 Months (Potential extension/Conversion FTE)

Location: Remote - FL (Region 5) - Pasco & Pinellas


Monday - Friday 8 am - 5pm


  • Day to day operations as a BH Case Manager
  • Reports directly to BH Supervisor and BH Manager but attends regional team meetings on a weekly and/or monthly basis and works cooperatively with the regional Supervisor(s) and Manager regarding caseload balancing and BH needs.
  • The BH SME will manage the needs of their assigned caseload, attend BH Staffings, assist in coordination of care, work collaboratively with any providers/stakeholders in volved with the member and their family and be able to document succinctly in our case management platform all interactions and correspondence.
  • This is a very independent role wrapped in team and leadership support to ensure best practices, ethical decision making and critical thinking is always in the forefront to ensure service coordination and delivery.
  • You will serve as an expert and provide guidance and education to your regional teammates regarding complex BH cases. You will serve as an expert in knowing how to navigate the regional system of care and work with the Community Base Care Agency in your region.
  • You will be supported by a Team of BH SMEs as well as BH Leadership have an active regional connection to the region you reside (participation in weekly and/or monthly regional team meetings)
  • Participation in weekly to biweekly team meetings with the BH Team
  • Supervision /Training / Resources provided by the BH Supervisor and Manager



Candidate Requirements
Education/Certification Required: Social Work, Psychology, Mental Health, Counselor Education Preferred: Social Work, Psychology, Mental Health, Counselor Education
Licensure Required: LCSW or LMHC Preferred: LCSW or LMHC

Must haves: At least 5 years' experience working in the field with children and families and the system of care. Excellent computer skills in Microsoft, able to learn and navigate various platforms.

Nice to haves: Qualified Supervisors; Clinical Supervisor experience, experience with DCF, DJJ, APD, DOH and /or a CBC or ME.

Disqualifiers: N/A

Performance indicators: able to multitask, manage high needs members, manage children and family members with mental health illness; provide excellent customer service; partner and work collaboratively with all providers/stakeholders. Manage challenging cases with multiple barriers.

1 Excellent Clinical Documentation and able to present cases succinctly on various platforms/teams.
2 Understanding and able to navigate the system of care/ connect with in-network providers and have an excellent understanding of the region/circuit processes for staffing members assigned to our plan.
3 Excellent computer skills - able to utilize multiple systems/platforms
