Title: Senior Care Navigator

Location: Remote

Duration: 3 months (potential to extend/convert)

Job Description:

Intake assessments on CMS members (telephonic case management)
Set tasking of assigned intake caseload with membership between 150 - 200 members
Virtual with a couple of in person meetings per year
Remote/Autonomous high tech work environment with assessments and telephone contact of members
Predominantly tier 4

Top 3 must-have hard skills Level of experience with each Stack-ranked by importance Candidate Review & Selection	1	2 Years of Pediatric Experience
2	Technology including Microsoft and EMR
3	Resource providing

Education/Certification	Required: As long as have LPN- no EDU minimum	Preferred: None
Licensure	Required: LPN active in Florida	Preferred: None
