Our rapidly growing Transplant Division is seeking a compassionate and highly organized Social Work Resource Assistantto join our dynamic, multidisciplinary team. This is a unique opportunity to support patients and families through some of the most critical moments of their transplant journey. In this role, you'll work closely with transplant social workers, coordinators, and clinical teams to ensure patients have timely access to essential psychosocial resources, education, and support services. This job is eligible for the federal Public Service Loan Forgiveness (PSLF) program. The Resource Specialist serves as a liaison between hospital and community agencies. The goal of the position is to provide patients and families with assistance to enable them to have access to healthcare. More specifically, assistance in the form of community options, agencies and concrete resources. PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS:
