Responsibilities

POSITION SUMMARY:

Manages the performance of all CM staff duties. Assists with the management of discharge planning from the beginning of the admission through to transition to next level of care. Also manages performance of all CM campus staff activities and work schedules. Responds to the daily operations of the departments; maintains a staffing plan that meets organizational needs; Maintains a system for reporting results. Responsible for the CM staff development; Works with the hiring ,firing, orientation ,training and evaluation according to established guidelines; Manages the quality control and performance improvement activities; Assures compliance with productivity and budgetary standards; Assures compliance with all Regulatory Agencies; Assesses develops and evaluates departmental goals on a yearly basis; Communicates departmental needs and recommends Capital equipment purchases; Will work closely with the Corporate and System Leaders as well as Administration in developing and implementing standards of work for the STHS facilities; and other duties as assigned. Qualifications

QUALIFICATIONS:

1. Minimum of 3 years experience in Case Management or related medical field.

2. A current Texas RN License or BSW/LMSW is required .

3. Current CPR certification

4. Knowledge of performance standards as outlined Centers for Medicare & Medicaid Service

5. Membership in related professional organization preferred

6. Must demonstrate commitment and adherence to STHS's Compliance Program and Code of Conduct through compliance with all policies and procedures, the Code of Conduct, attendance at required training and immediately reporting suspected compliance issue(s) to the Compliance Officer.

7. Strong interpersonal skills and experience in team development.

EDUCATION / LICENSURE:

1. Registered Nurse or BSW/LMSW currently licensed in the State of Texas, or part of a compact state license, or has a valid temporary RN license for the State of Texas while waiting to receive the permanent license. BSN or LMSW preferred.

2. Must possess an Associate Degree as an RN, a Baccalaureate Degree in Social Work or Master Degree in Social Work or Diploma from a School of Nursing or University.
