Position Summary

The RN Sepsis Clinical Coordinator is responsible for the implementation of day-to-day clinical activities and assist with the development of all initiatives related to the MemorialCare Sepsis Program at the Long Beach Medical Center. Performs concurrent data collection, sepsis database management, and performance improvement initiatives based on the data results and management recommendations, clinically intervening as needed. Supports management in achieving a high-quality program, for both clinical outcomes and patient/family service excellence. Supports MemorialCare's strategic plan of becoming a High Reliability Organization (HRO) at Long Beach Memorial Medical Center. **Success is defined but not limited to; a reduction in sepsis mortality rates, decrease in sepsis related re-admissions, improved CMS perfect care compliance, and achievement of the Bold Goal.

Essential Functions and Responsibilities of the Job

  • Collects, analyzes, and disseminates sepsis program patient data to appropriate groups.
  • Assisting with documentation, coding, clinical care and improved outcomes.
  • Facilitates the continual development, implementation, refinement and ongoing adherence of sepsis program protocols and therapies and promotes adoption of national standards and best practices in management of sepsis patients.
  • Writes, reports, and maintains documentation of activities related to programs objectives.
  • Routinely meets with the Physician champions.
  • Ensures that protocols and therapies become the standard of care followed within the facility.
  • Provides education and clinical expertise to care teams, patients, and families. **Early identification and treatment of septic patients.
  • Participates in patient rounds and proactively responds to patient needs to improve patient outcomes.
  • Collects, analyzes, and presents sepsis data to ED Sepsis Workgroup, Inpatient Sepsis Workgroup, and the Sepsis Oversight Committee. Continuously adjusting workload (as needed) and collaborating with the CMO & Quality Team.
  • Works closely with the Code Team and Rapid Response Team, spearheading process improvement initiatives for earlier recognition of sepsis.
  • Performs other duties as assigned.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications.
