Job Summary:

This position is accountable to assist patients and their families with managing a cancer diagnosis and overcoming barriers to timely and appropriate cancer care. The Associate in this position will support the mission, vision, values and strategic initiatives of Mary Washington Healthcare and must demonstrate a commitment to quality service to patients, physicians, the public and co-workers. The Associate will be responsible for compliance with all federal and state laws and regulations governing the privacy of patients' protected health information

Essential Functions & Responsibilities:

  • Serve as the point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive cancer care services offered within Mary Washington Healthcare and in the community.
  • Respond to consults from referring physicians or self-referrals, and provide consultation for patients with suspicious findings of cancer, or a confirmed diagnosis, within 24 hours.
  • Assist patients in understanding their diagnosis, treatment options, clinical trial availability, and the supportive resources available.
  • Communicate relevant information among practitioners and across settings, sharing participant information in a timeframe that meets participant's needs, and confirming practitioner's receipt of information.
  • Provide and reinforce patient/caregiver education relative to the cancer diagnosis and supportive care resources that are available through the Regional Cancer Center.
  • Establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, and social services staff).
  • Provide support for Cancer Program development, working as directed and in collaboration with Regional Cancer Center leaders.
  • Collect data and track outcomes, identifying trends and opportunities for program improvement.
  • Facilitate site-specific cancer conferences and attend weekly cancer conferences as appropriate.
  • Deliver quarterly reports to the Cancer Committee, Clinical Teams, and other groups as indicated, demonstrating outcomes and performance improvement activities.
  • Maintain a broad and current knowledge base of cancer care and institutional cancer data. Interface effectively with medical staff, other departments, and community representatives.
  • Document coordination of care appropriately in the medical record.
  • Facilitate appointments for specialty physician consults, diagnostic services, and support services, including social workers, dietitians, cancer rehab, genetic counseling, etc.
  • Act as a liaison for patients and families, assisting in the transition from sequential care through diagnosis and treatment, and facilitate overcoming any barriers to care.
  • Serve as resource to the community on cancer prevention and screening through public awareness and education and participation in community screenings.
  • Perform other duties as assigned.

Qualifications:

  • Registered Nurse licensed to practice in the state of Virginia.
  • Oncology experience preferred.
  • Two years clinical experience preferred.
  • Must be able critically think, resolve problems and work with diverse personalities and situations.
  • Must have excellent interpersonal skills, and be proficient in verbal and written communication.
  • Prior experience working with physicians preferred.
  • Must have experience with computerized systems. Must be able to work independently
  • Must be able to collaborate with physicians, staff, and MWHC Associates.
