Tuberculosis Navigation Coordinator

POSITION SUMMARY:

Performs the key duties of the Tuberculosis (TB) Navigation Program including assisting patients with pre-appointment care coordination and follow up arrangements, manage referrals from BMC providers, and completing prior authorizations when necessary. Manages various aspects of program administration and operation, including reporting on Tuberculosis Navigation Program financial and outcomes, program planning and design, public relations and promoting the program to referring programs and agencies. Duties include managing data, preparing grant reports; functioning as a key liaison with public and private funding sources, regulatory agencies and relevant hospital programs.

ESSENTIAL RESPONSIBILITIES / DUTIES:


  • Meet with patients in the Tuberculosis Clinic at Boston Medical Center during their scheduled clinic visits to provide healthcare system navigation support, including booking appointments in other departments, assisting with transportation arrangements, and facilitating prescribed medications/treatments

  • Review upcoming clinic schedule to ensure that new patients have completed imaging and/or lab work ahead of appointments and have adequate transportation

  • Manage referrals received through Epic to ensure necessary orders have been placed ahead of the visit and that patient is appropriate for treatment

  • Assist in rescheduling patients as needed in case of provider or nurse schedule changes

  • Support reporting to funders by providing data and context about key performance indicators

  • Manage spending related to grant activities including transportation costs, grocery store cards, and patient supplies

  • Build PowerPoint presentations, oral presentations, posters, pamphlets, fliers, etc as needed to support education about the TB clinic and facilitate patient education

  • Track key clinical and operational metrics for grant reporting and department performance

  • Experience with Windows, Word, Excel, Outlook, Internet and web page usage/updating, social networking and other technologies that can be used to further carry the program's mission.

  • Demonstrated skills in the provision of services in a multicultural environment.


JOB REQUIREMENTS

EDUCATION:


  • High School Diploma required

  • Bachelor's Degree preferred (health studies, public health, human services, social work, research, or related field preferred).


EXPERIENCE:


  • Must have a minimum of three to five years of healthcare administrative experience.

  • Prior Healthcare Navigator experience preferred.


KNOWLEDGE AND SKILLS:


  • Additional language skills (beyond that of English) appropriate to the patient population served is very helpful. Haitian Creole, Spanish, and/or Vietnamese strongly preferred.

  • Excellent oral and written communication skills; ability to be detailed oriented in all notes and documentation.

  • Demonstrated ability and experience in areas such as training, team building, program development/management, problem solving, and community building.

  • Ability to analyze needs and apply resources effectively to meet those needs.

  • Ability to achieve targeted results.
