Job Summary

Utilizes specialized knowledge and skills within major areas of focus, such as patient / family, nurses and nursing practice, and the organization / system. Provides specialized clinical education in regards to treatment and liaisons with the patients clinical service providers to address specific concerns. Has in-depth specialty expertise and focus.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $148,800 - $223,200 (Annual Rate).

Department Description

St. Mary's Medical Center and associated outpatient clinics are now part of UCSF Health. This addition give us the opportunity to offer primary and secondary care in more places and increases UCSF Health's ability to care for patients with complex medical needs. Combining community care with academic medicine makes convenient, comprehensive care easier to access for all our patients.

Required Qualifications

• Must have greater than two years of clinical experience in an acute care hospital. Palliative care, Oncology, Hospice, or home care experience required.
• Must have knowledge of death and the dying process, terminal/chronic illness, grief, coping skills, and pain and symptom management. Knowledge of community resources related to palliative care. Knowledge of regulatory requirements for palliative care.
• Advanced skills and expertise working in a complex, multi-divisional environment, with proven ability to initiate projects and lead teams, prioritize tasks, and direct projects successfully from inception to completion on task and on schedule.
• Advanced communication skills to teach patients about complex, sensitive clinical issues.
• Demonstrated knowledge with Windows-based software including Microsoft word, Excel, Outlook. Strong Knowledge of clinical based computer systems and software.

Preferred Qualifications

• Bilingual Cantonese/ English speaking
• Advanced Practice Nurse (CNS or NP), Certified Hospice and Palliative Care Nurse (CHPN) CHPN (No c for care) or a Certified Hospice & Palliative Social Worker (CHP-SW) LCSW (Licensed Clinical Social Worker) Or an Advance Certified Hospice and Palliative Social Worker - ACHP-SW

License/Certification

• Bachelor's degree in related area and / or equivalent experience / training.
• RN or MSW
• Requires clinical licensure in relevant discipline.
• Valid California, RN, BSW, MSW License
• American Heart Association BLS
